Atrophy
Atrophy is a crafted legendary wand that can only be created in Reaper of Souls. After Patch 2.0.6 this item no longer requires a Phial of Weakness or any other legendary material to craft.
To create this item, find the Plan: Atrophy, which can be randomly dropped randomly by any level 70 target, and teach the plan to the Blacksmith. Once learned by the smith, any character on your account can make as many of these as you can afford to create, though only the Wizard can equip it.

Atrophy stats
This crafted wand has no legendary affix and a damage range in line with other wands, so it's not as good as some other legendary or set options. It can roll bigger numbers than rares though, and many new Wizards will use it until they find something higher end.

Once a player finds the randomly-dropped plan and teaches it to the Blacksmith, the item can be created by any character on that account. This item always has a lvl 70 requirement, no matter what level character crafts it. The Smart Loot system means that this item will usually roll with a mainstat and/or +skill damage bonus appropriate to the class of the character that creates it.

Patch 2.0.6 controversially[1] removed Legendary Materials, which changed the crafting costs for every level 70 RoS recipe. Prior to that patch Atrophy required 1 Phial of Weakness and 1 Death's Breath.

Legendary Material
Legendary Materials were controversially[2] removed in Patch 2.0.6. The following is archived for posterity.
The required legendary material, Phial of Weakness is dropped only by Unique (purple) Heralds of Pestilence. Commonly called "ticklers," they are found in Act , and attack by sending one hand into the ground, with the fingers emerging some distance away and dealing poison damage. Blighter the Rift Guardian is a monster of this type, but he can not drop the legendary material.
